This increased pressure in the veins, known as venous hypertension, is the primary driver of PTS symptoms [1].

- **Damage to Venous Valves:** The inflammatory response triggered by the blood clot can destroy or impair the delicate one-way valves within the veins. This valve dysfunction is a hallmark of PTS.
- **Persistent Venous Obstruction:** Even after the initial DVT has been treated, residual clot material can partially or completely obstruct the vein, leading to continued high pressure and impaired blood flow.
- **Vein Wall Damage:** The DVT can also cause direct damage to the vein wall, leading to scarring and stiffening, which further compromises venous function.

- **Lipodermatosclerosis:** Hardening and thickening of the skin and underlying fat, giving the leg a inverted champagne bottle’ appearance. This can be a sign of advanced venous disease.
- **Venous Eczema:** Red, itchy, and scaly skin.
- **Venous Ulcers:** In severe cases, open sores (ulcers) can develop, typically around the ankle, which are often painful and slow to heal.
Diagnosing Post-Thrombotic Syndrome
Diagnosis of PTS is primarily clinical, based on a patient's history of DVT and the presence of characteristic symptoms. Healthcare professionals often use scoring systems, such as the Villalta scale, to assess the severity of PTS. Imaging techniques, such as duplex ultrasound, can help evaluate venous reflux and obstruction, providing objective evidence of venous damage [3].
Management Strategies for Post-Thrombotic Syndrome
The management of PTS focuses on alleviating symptoms, preventing progression, and improving quality of life. Treatment approaches are often multifaceted and tailored to the individual patient's needs.
Conservative Management
Conservative measures are the cornerstone of PTS management and are recommended for most patients:
- **Compression Therapy:** This is arguably the most critical component of PTS treatment. Graduated compression stockings (GCS) apply pressure that is highest at the ankle and gradually decreases up the leg, helping to reduce swelling, improve venous return, and alleviate pain. - **Leg Elevation:** Elevating the affected leg above heart level, especially during rest, can help reduce swelling and discomfort.

- **Weight Management:** For overweight or obese individuals, weight loss can significantly reduce pressure on the leg veins and improve symptoms.
Pharmacological Interventions
While there is no specific medication to cure PTS, certain drugs may be used to manage symptoms:
- **Pain Relievers:** Over-the-counter pain medications (e.g., NSAIDs) or prescription analgesics may be used for pain management.
- **Diuretics:** In some cases, diuretics may be prescribed to help reduce severe swelling, though their use should be carefully monitored.
