Pulmonary Embolism (PE)
Pulmonary embolism occurs when a blood clot — usually from a leg vein — travels to the lungs and blocks a pulmonary artery. It strains the heart and lowers oxygen levels, and a large PE is life-threatening without rapid treatment.
Overview
Most pulmonary emboli begin as deep vein thrombosis. When a fragment breaks free, it rides the venous bloodstream through the right heart and lodges in the pulmonary arteries. Small emboli may cause only breathlessness; large or multiple emboli raise the pressure the right ventricle must pump against and can cause collapse. Modern PE care is organized around risk: low-risk patients often need anticoagulation only, while intermediate and high-risk patients may benefit from catheter-based clot removal in specialized centers.
Signs & Symptoms
- Sudden shortness of breath at rest or on minimal effort
- Sharp chest pain that worsens with deep breathing
- Rapid heart rate or palpitations
- Cough, occasionally with blood-streaked sputum
- Lightheadedness or fainting — a warning sign of severe PE
- Signs of DVT: one-sided leg swelling and pain
How It Is Diagnosed
CT pulmonary angiography is the definitive test, showing clots within the lung arteries. D-dimer blood testing helps exclude PE in low-probability patients. Echocardiography assesses strain on the right heart, and leg ultrasound identifies the source clot. Risk scores combine these findings to guide where and how aggressively to treat.
Treatment Options
Anticoagulation
Immediate blood thinning stops clot growth while the body clears the embolism. It is the core treatment for the majority of PEs.
Systemic thrombolysis
For massive PE with shock, clot-dissolving medication given intravenously can be lifesaving, at the cost of higher bleeding risk — reserved for the most severe cases.
Catheter-directed thrombolysis
A catheter placed into the pulmonary artery delivers low-dose thrombolytic directly into the clot, relieving right-heart strain in intermediate–high-risk PE with less systemic bleeding exposure.
Aspiration (suction) thrombectomy
Catheter systems extract clot mechanically without thrombolytic drugs — an option when bleeding risk is high or rapid unloading of the right ventricle is needed.
IVC filter protection
When anticoagulation is contraindicated, a retrievable inferior vena cava filter can intercept further emboli traveling from the legs until treatment can resume.
When to See a Doctor
Sudden unexplained breathlessness, chest pain with breathing, or fainting requires emergency care — call an ambulance. If you have a known DVT and develop any breathing symptoms, treat it as an emergency.
