Deep Vein Thrombosis (DVT)
Deep vein thrombosis is a blood clot that forms in a deep vein, most often in the leg or pelvis. It can cause sudden swelling and pain, and if part of the clot travels to the lungs it becomes a pulmonary embolism — a medical emergency.
Overview
Deep veins lie within the muscles and carry most of the blood returning to the heart. A clot forms when blood flow slows (long immobility, surgery, casts), the vessel wall is injured, or blood has an increased tendency to clot (genetics, cancer, hormones, pregnancy). The clot blocks drainage from the limb, causing swelling and pain. Beyond the immediate risk of pulmonary embolism, a clot that heals with scarring can permanently damage vein valves, leading to chronic swelling and skin changes known as post-thrombotic syndrome.
Signs & Symptoms
- Swelling of one leg (or arm), often rapid in onset
- Cramping or aching pain, typically in the calf
- Warmth and redness or discoloration of the skin
- Visible surface veins
- Sudden shortness of breath or chest pain may signal pulmonary embolism — call emergency services
How It Is Diagnosed
Duplex ultrasound is the standard first test and shows whether a deep vein is compressible and whether flow is blocked. Blood tests such as D-dimer help rule out clots in low-risk patients. CT venography or MR venography maps clots in the pelvis or abdomen when ultrasound cannot reach them.
Treatment Options
Anticoagulation (blood thinners)
The foundation of DVT care. Anticoagulants stop the clot from growing and let the body dissolve it over months. Duration ranges from three months to indefinite depending on the cause and recurrence risk.
Compression therapy
Graduated compression stockings reduce swelling and may lower the risk of post-thrombotic syndrome in symptomatic patients.
Catheter-directed thrombolysis
For extensive clots — especially iliofemoral DVT — a catheter delivers clot-dissolving medication directly into the thrombus, used in selected patients to relieve obstruction faster than anticoagulation alone.
Mechanical and pharmacomechanical thrombectomy
Catheter systems physically remove or fragment the clot, sometimes combined with local thrombolytic drug delivery, restoring flow in a single session for appropriately selected patients.
Venous stenting
When a compressed or scarred iliac vein underlies the clot (such as May-Thurner syndrome), a dedicated venous stent can hold the vein open after clot removal.
IVC filters
A retrievable filter in the inferior vena cava can intercept traveling clot fragments when anticoagulation is temporarily impossible or has failed. Filters are removed when no longer needed whenever feasible.
When to See a Doctor
Sudden one-sided leg swelling and pain deserves same-day medical review. Chest pain, breathlessness, or fainting alongside leg symptoms is an emergency — call an ambulance immediately.
