"You'll need ORIF surgery" is a sentence many patients hear for the first time in an emergency department, often without much explanation of what the acronym actually means. ORIF stands for Open Reduction Internal Fixation, a surgical approach used to treat certain fractures by surgically exposing the bone, realigning the broken fragments, and holding them in place with implanted hardware. It is one of the most established techniques in orthopedic trauma care and is used across a wide range of fracture types, from long bones like the femur and tibia to smaller bones in the wrist or ankle.

Breaking Down the Term: Open Reduction and Internal Fixation

The name describes the procedure in two parts, and understanding each half clarifies what actually happens in the operating room.

Open reduction refers to surgically opening the area around the fracture so the surgeon can directly visualize and manipulate the bone fragments back into proper alignment. This is distinct from "closed reduction," where a fracture is realigned without surgical incision, typically followed by a cast rather than implanted hardware.

Internal fixation refers to the hardware used to hold the realigned bone in place from within — plates, screws, or intramedullary nails inserted directly at or into the bone, as opposed to external fixation devices that stabilize the bone from outside the skin using pins and an external frame.

Together, ORIF surgery means the fracture is both directly realigned and internally secured, generally intended to allow the bone to heal in the corrected position.

When Do Surgeons Recommend ORIF Instead of a Cast?

Not every fracture requires surgery. A qualified physician generally considers ORIF when a fracture is displaced (the bone ends are out of alignment), unstable, involves a joint surface, or is unlikely to heal properly with casting alone. Common scenarios include:

  • Fractures where bone fragments have shifted significantly out of position
  • Fractures extending into a joint, where precise alignment affects future joint function
  • Certain fractures in bones subject to high mechanical load, such as the femur or tibia
  • Fractures that failed to stabilize adequately with a cast or splint alone

The decision to proceed with ORIF versus non-surgical management depends on fracture pattern, patient age and activity level, bone quality, and overall health — it is an individualized clinical judgment rather than a fixed rule tied to any single fracture type.

What Types of Hardware Are Used in ORIF Surgery?

Internal fixation hardware varies by fracture location and pattern, and orthopedic trauma systems generally fall into a few categories:

  • Plates and screws — a plate is contoured to the bone's surface and secured with screws, commonly used for fractures near joints or in bones where a rigid external surface fixation makes sense. Locking plate systems use screws that thread into the plate itself, providing angular stability that can be particularly relevant in osteoporotic bone, while non-locking (compression) plates rely on friction between the plate and bone surface.
  • Intramedullary nails — a rod inserted through the hollow center (medullary canal) of a long bone, generally used for shaft fractures of bones like the femur or tibia. Recovery timelines vary considerably by fracture location and severity, but ORIF surgery is generally followed by a period of restricted weight-bearing or activity, progressing gradually under the guidance of the surgical team and, in many cases, physical therapy. Follow-up imaging is typically used to confirm the fracture is healing in the corrected alignment before activity restrictions are eased further. Because every fracture and patient is different, only the treating surgeon can provide an individualized recovery timeline.

Is Hardware From ORIF Surgery Permanent?

In most cases, internal fixation hardware is designed to remain in place indefinitely once the fracture has healed, since titanium and titanium alloy implants are generally biocompatible and well tolerated long-term. Removal is not routine and is typically considered only if hardware becomes symptomatic, causes irritation near the skin surface, or if removal is otherwise clinically indicated. This decision is made on a case-by-case basis between patient and surgeon.
