Reading a Coronary Angiogram: A Patient-Friendly Guide

"What did the angiogram show?" is one of the first questions many patients ask after a cardiac catheterization procedure, and the black-and-white images on the screen can look confusing without some context. A coronary angiogram is an X-ray-based imaging test that uses contrast dye injected into the coronary arteries to show whether, and where, narrowing has developed inside the vessels that supply blood to the heart muscle. This guide walks through the basics of angiography, how stenosis percentage is estimated, and how a treating cardiologist interprets what appears on the images, so patients have a clearer sense of what the report and pictures actually represent.

What Is Angiography and How Does It Work?

Angiography basics start with a thin catheter, typically inserted through an artery in the wrist or groin, that is guided under X-ray (fluoroscopy) guidance to the opening of the coronary arteries. Once positioned, the physician injects a small amount of iodine-based contrast dye directly into the artery. Because contrast dye is denser than blood and blocks X-rays more than surrounding tissue, it outlines the inner channel of the vessel in real time, creating a moving image that shows the path, caliber, and any areas of narrowing along each coronary artery. Multiple views from different angles are typically captured so that a narrowing is not missed or overestimated due to the angle of a single image.

If a vessel that would normally measure a certain diameter narrows to roughly half that width at one point, this is typically described as an approximately 50 percent stenosis, and so on for other degrees of narrowing. This assessment can be made visually by an experienced operator or supported by quantitative coronary angiography software that measures vessel edges more precisely. Higher-grade narrowings, commonly those in a more severe range, are more likely to be flagged as candidates for treatment, though the decision also depends on the patient's symptoms and additional testing.

Why Doesn't a Percentage Alone Decide Treatment?

A stenosis percentage is only one piece of information a cardiologist weighs. Additional factors commonly considered include whether the narrowing is limiting blood flow in a way that correlates with the patient's symptoms, results from supplementary tests such as fractional flow reserve measurements, the specific vessel and segment involved, and the patient's overall cardiac history and risk factors. Two patients with a similar-looking percentage of narrowing on an angiogram may be managed differently depending on these broader clinical circumstances, which is why the angiogram is interpreted alongside the full clinical picture rather than in isolation.

What Do the Different Views and Terms Mean?

Angiogram reports often reference terms such as "proximal," "mid," and "distal" to describe where along a vessel's length a finding is located, moving from closer to farther from the vessel's origin. Terms like "diffuse disease" describe narrowing spread across a longer segment rather than one discrete point, while "focal lesion" describes a narrowing concentrated in a short segment. Reports may also note vessel tortuosity, calcification, or the presence of collateral vessels, which are alternate small pathways that can supply blood flow around a blocked segment.
