Post-thrombotic syndrome (PTS) is the chronic sequela of deep vein thrombosis, resulting from a combination of persistent venous obstruction and valvular reflux. It ranges from leg heaviness and swelling to disabling pain and venous ulceration, and it significantly affects quality of life after iliofemoral DVT.
Pathophysiology
After DVT, incomplete recanalization leaves residual obstruction while damaged valves permit reflux. The combination raises ambulatory venous pressure, driving the edema, skin changes, and ulceration characteristic of advanced disease. Iliofemoral obstruction is a particularly strong contributor to severe PTS.
Patient selection for stenting
The best candidates for venous stenting have symptomatic, hemodynamically significant iliofemoral or iliocaval obstruction confirmed on cross-sectional imaging and, ideally, intravascular ultrasound. Selection weighs symptom severity, the extent of obstruction, inflow quality, and patient factors. Reflux-dominant disease without significant obstruction is less likely to benefit from stenting alone.

- IVUS assessment defines lesion length and degree of obstruction and guides stent sizing.
- Dedicated venous stents provide the diameter, crush resistance, and flexibility required for the iliocaval segment.
- Adequate inflow from the femoral confluence is essential for durable patency.
Outcomes and management
Reported series show meaningful improvement in pain, swelling, and ulcer healing after successful stenting of obstructive lesions, with patency supported by antithrombotic therapy and surveillance. Conservative measures — compression therapy, skin care, and exercise — remain foundational for all PTS patients, whether or not stenting is performed.
