Feeling Plates Under the Skin: Causes and Options

Can you feel plates and screws under the skin? Often yes, especially near bony areas. Causes, when it matters, and options if it becomes bothersome.

"Is it normal to feel the metal under the skin?" is a question that comes up often enough during follow-up visits after fracture surgery that it is worth addressing directly. The short answer is yes — many patients can feel plates and screws to some degree, particularly over bony areas where there is little soft tissue cushioning between the implant and the skin surface. Whether this is something to be concerned about, and what can be done if it becomes bothersome, depends on the location, the degree of discomfort, and how the hardware is affecting daily activity.

Why Is Hardware More Noticeable in Some Locations Than Others?

The amount of soft tissue covering a bone varies dramatically across the body, and this is the single biggest factor in whether implanted hardware is palpable. Areas with thin muscle and fat coverage over the bone — such as the ankle, the front of the shin, the clavicle (collarbone), the outer elbow, and the top of the foot — tend to have hardware sit closer to the skin surface, making plates and screws more likely to be felt or even visible as a subtle contour change. By contrast, hardware placed in areas with thicker surrounding muscle, such as the thigh or upper arm, is generally less noticeable because more tissue separates the implant from the skin.

This is simply anatomy, not a sign that something went wrong with the surgery. Surgeons account for expected soft tissue coverage when selecting plate design and screw length, but some degree of prominence is often unavoidable in thin-coverage areas.

When Does Palpable Hardware Cross Into a Problem?

Feeling hardware under the skin is not, by itself, a medical problem. It becomes a more relevant clinical issue when it is accompanied by:

  • Pain or tenderness when pressure is applied to the area, such as from clothing, shoes, or bracing
  • A sensation of the hardware catching on a tendon during movement
  • Skin irritation, redness, or thinning of the skin directly over the implant
  • Difficulty with certain footwear, straps, or equipment due to hardware prominence

Any of these signs are worth raising with the surgical team, since they may indicate that the hardware is mechanically irritating surrounding soft tissue rather than simply being palpable at rest.

What Options Exist If Prominent Hardware Becomes Bothersome?

For patients whose palpable hardware is asymptomatic — noticeable but not painful or limiting — no intervention is typically needed, since prominence alone is a cosmetic or sensory finding rather than a medical one. When hardware does become symptomatic, options a surgeon might discuss include padding or footwear adjustments for external pressure points, activity modifications, or in some cases, elective hardware removal once the fracture has fully healed. The decision to pursue removal for symptomatic prominence follows the same individualized evaluation process used for any hardware removal decision, weighing the benefit against the risks of an additional procedure.

Do Low-Profile Plate Designs Reduce This Issue?

Plate design does influence how prominent an implant feels. Low-profile plates are designed with a thinner cross-section and more anatomically contoured shape intended to sit closer to the bone surface, which can reduce (though not necessarily eliminate) the sensation of hardware prominence in thin-coverage areas. Is Feeling Hardware a Sign the Implant Is Failing?

Generally, no. Palpability alone, without pain, mechanical symptoms, or skin changes, is not typically a sign of hardware failure or malposition. That said, a new change in how hardware feels — such as sudden new pain, movement or shifting sensation, or skin breakdown over the implant — should prompt evaluation by the surgical team, and any sign of infection, such as redness, warmth, drainage, or fever, should prompt seeking immediate medical care. Does feeling hardware mean it will eventually need to come out?

Not necessarily. Many patients feel their hardware indefinitely without it ever becoming symptomatic enough to warrant removal. The decision to remove hardware depends on symptoms and functional impact, not on palpability alone.
