Peripheral Arterial Disease (PAD)
Peripheral arterial disease is the narrowing of arteries that supply the legs, usually from atherosclerosis. It causes cramping pain on walking, and in advanced stages pain at rest or non-healing wounds — signs the limb needs urgent revascularization.
Overview
The same plaque-forming process that narrows heart arteries also affects the leg arteries. As narrowing progresses, muscles receive too little blood during exercise, producing the hallmark cramping pain called claudication that resolves with rest. Critical limb ischemia — rest pain, ulcers, or gangrene — means blood flow is barely sufficient for the tissue at rest and carries a high risk of amputation without revascularization. PAD also marks a patient as high-risk for heart attack and stroke, so treating the whole cardiovascular system matters as much as treating the leg.
Signs & Symptoms
- Cramping calf, thigh, or buttock pain when walking that eases with rest
- Cold feet or a weaker pulse in one leg
- Wounds or ulcers on the foot that heal slowly or not at all
- Rest pain in the toes or foot, often at night, relieved by hanging the leg down
- Shiny skin, hair loss on the legs, thickened toenails
- Color changes — pallor on elevation, redness when hanging down
How It Is Diagnosed
The ankle-brachial index compares blood pressure at the ankle and arm — a simple, painless screening test. Duplex ultrasound locates narrowings; CT or MR angiography maps the entire arterial tree; catheter angiography provides definitive imaging at the time of treatment.
Treatment Options
Risk-factor treatment and exercise therapy
Smoking cessation, statins, blood pressure and diabetes control, antiplatelet therapy, and structured walking programs are the foundation — they improve walking distance and protect heart and brain.
Balloon angioplasty (including drug-coated balloons)
A balloon catheter opens the narrowed segment from inside the artery. Drug-coated balloons leave an anti-restenosis medication in the vessel wall to help keep it open.
Stenting
Self-expanding stents scaffold arteries that recoil or dissect after ballooning, restoring a durable channel — commonly used in the iliac and femoral arteries.
Atherectomy
Rotational or directional catheters debulk hard, calcified plaque, preparing the vessel so balloons and stents work better, particularly in heavily calcified disease.
Bypass surgery
For long occlusions unsuitable for endovascular repair, a surgical bypass using the patient’s own vein or a graft reroutes blood around the blockage.
When to See a Doctor
Walking pain that limits daily life warrants a vascular assessment. Foot wounds that do not heal within two weeks, blackened skin, or pain at rest are urgent — early revascularization saves limbs.

Do I need open surgery for PAD?+
Most patients are treated endovascularly — through a needle puncture — with balloons, stents, or atherectomy. Surgery remains valuable for specific patterns of disease; many centers combine both in hybrid procedures.
